About After65 Forms

After65 Forms is a small, independent service run by Northpine Labs LLC. We help people around retirement age with two pieces of Medicare paperwork that are easy to get wrong and costly to miss: signing up for Part B through the Special Enrollment Period when job-based coverage ends (forms CMS-40B and CMS-L564), and asking Social Security to lower IRMAA after a life-changing event (form SSA-44).

Not affiliated with the Social Security Administration, CMS or Medicare. The forms are free at ssa.gov and cms.gov; we help you prepare them correctly. You can always do this yourself for free, or get free one-on-one help from Social Security and your State Health Insurance Assistance Program (shiphelp.org). Our tools give you a free answer first; the optional paid kits fill in the official forms with your details and add the checklists, letters and steps that make the request go smoothly.
