Signing up for Part B when employer coverage ends

If you (or your spouse) kept working past 65 with a job-based health plan, you can sign up for Medicare Part B later without a penalty, but only inside a Special Enrollment Period with a hard deadline. Answer a few questions to see whether you qualify, your exact last day, and which forms you need. It's free.

If you qualify, the $39 kit fills in the CMS-40B and CMS-L564 for you, with instructions for your employer and a backup plan if they won't sign. Or follow our free guides: the 8-month window explained.

1.Your date of birth

Used only to work out your Medicare eligibility month. It stays in this browser.

2.What health coverage do you have now (or had most recently), besides Medicare?
3.Do you already have Medicare Part A?

Most people get premium-free Part A. Your red, white and blue Medicare card shows it.

Your answers stay in this browser. Nothing is sent to our server.
