Privacy
Short version: your answers stay on your device. After65 Forms is run by Northpine Labs LLC. We built the tools so that we do not need, and do not keep, your personal information.
What stays in your browser
Everything you type into the questionnaires and the form details (your name, address, dates, employer and income figures) is saved only in your own browser's local storage, so it is still there when you come back from the payment page. It is not sent to our server. The filled PDF forms are created inside your browser. You can erase the saved answers at any time with the "Clear my answers" button on each tool, or by clearing your browser's site data.
We never ask for your Social Security or Medicare number
The official forms have boxes for these numbers. We leave them blank on purpose, and you write them in by hand before you sign. That way the numbers never pass through any website.
Payments
Payments are handled by Stripe. Stripe collects your card details and e-mail address for the receipt under its own privacy policy; we never see your card number. We do not send Stripe any of your questionnaire answers: the payment record only says which kit you bought.
Analytics
We use PostHog to count visits and see which steps people complete (for example "questionnaire finished" or "checkout started"), so we can improve the tools. These events do not include your answers. Typed text is masked in any session recording. Our server logs keep standard technical data (IP address, browser type) for security for a short period.
Cookies
We use only the cookies and local storage needed for analytics and for keeping your answers between visits. No advertising cookies.
Your choices
Clear your answers on the tool pages, use your browser's private mode, or block analytics with your browser settings: the tools still work. Questions or requests: support@after65forms.com.
