What we are, and what we are not

After65 Forms is a document-preparation help service operated by Northpine Labs LLC. Not affiliated with the Social Security Administration, CMS or Medicare. The forms are free at ssa.gov and cms.gov; we help you prepare them correctly. We are not a law firm, a financial adviser, an insurance agent or a Medicare plan, and nothing on this site is legal, tax or financial advice. Social Security and Medicare decide your enrollment and premiums. Free one-on-one help is also available from Social Security (1-800-772-1213) and your State Health Insurance Assistance Program (shiphelp.org).

What you buy

  • Part B Special Enrollment Kit ($39): the official CMS-40B and CMS-L564 filled with the details you enter, an instruction sheet for your employer, an alternative-evidence checklist, a cover letter, submission steps and a deadline timeline.
  • IRMAA SSA-44 Kit ($39): the official SSA-44 filled with the details you enter, an evidence checklist for your life-changing event, a cover letter, submission steps and what happens next.
  • Both kits (Part B SEP + IRMAA SSA-44) ($59): both of the above.

Each is a one-time payment, not a subscription. The documents are generated in your browser from your own answers; you are responsible for checking that the information is correct before you sign and send anything. Your result is an estimate based on what you tell us and the published rules; Social Security makes the actual decision.

14-day full refund

If the kit does not help you, you get your money back. Within 14 days of purchase, use the "Refund my purchase" button on your kit page; it refunds the full amount to your card immediately, no questions asked. You can also e-mail support@after65forms.com with your purchase reference. Card refunds usually appear within 5 to 10 business days, depending on your bank.

Accuracy

We take the rules and dollar amounts from official SSA, CMS and Medicare publications and list each source with the date we checked it. Rules change; if we find an error we correct it promptly. Our liability is limited to the amount you paid.

Please use the tools only for your own (or your family member's, with permission) paperwork.
