FEHB and PSHB premiums for 2027: what retirees pay each month

Checked against official OPM, CMS and Social Security sources on November 1, 2026. Not affiliated with any government agency.

If you are retired and in FEHB or PSHB, your health premium comes out of your annuity every month. Here is what each plan costs in 2027, straight from OPM's 2027 rate tables, and what to add once Medicare Part B is in the picture. Open Season, the window to switch, runs November 9 to December 14, 2026.

What a retiree pays

  • The enrollee share, monthly. OPM's rate tables give two figures for each plan: the government share and the enrollee share. Employees see the enrollee share biweekly; retirees pay the monthly enrollee share, deducted from the annuity. OPM states its maximum government contribution for "most employees and annuitants" as one number, so most retirees pay the same share as an employee in the same plan.
  • Three enrollment types. Self Only covers you; Self Plus One covers you and one family member, usually a spouse; Self and Family covers everyone eligible. Self Plus One is not always cheaper than Self and Family: OPM flags the plans where it costs more.
  • The code matters. Each plan option has a three-character enrollment code per enrollment type. Your annuity statement shows yours; the tables below list each option's Self Only code to help you find your row.

What changed for 2027

OPM's 2027 Open Season Highlights put the average enrollee-share increase at 10.9% for FEHB and 8.2% for PSHB (overall premiums: 9.3% and 6.7%). Averages hide a wide spread: some plans rose much more, and a few went down. Compare your 2027 row with the deduction on your current annuity statement to see your own change. The Open Season 2027 guide covers the plans that are leaving and the other changes.

All figures: OPM's 2027 FEHB and PSHB rate tables, "Monthly, Employee/Annuitant pays", downloaded again and checked on November 1, 2026 (no change since October 27). The Panama Canal Area Benefit Plan is left out: it serves the Panama Canal area. Some plans also need a union membership (APWU, MHBP, NALC, Rural Carrier) or have other enrollment limits; the plan's brochure says which.

Add Medicare Part B if you have it

Part B is a separate Medicare premium, usually taken from your Social Security benefit, not your annuity. At the 2026 standard rate it is $202.90 a month per person, and more if your income is high enough for IRMAA. CMS hadn't published the 2027 amount when we checked on November 1, 2026; it usually does in November, and our 2027 IRMAA page switches to the official figures once it does.

A married couple on Self Plus One where both have Part B pays the plan's Self Plus One share once, plus two Part B premiums. For postal retirees, Part B may not be optional: see the PSHB Part B requirement.

The premium is only half the cost

A cheap plan can cost more over a year once you add its deductible and copays. With Medicare paying first, many plans drop those, and a few pay part of the Part B premium back. See which plans waive costs with Medicare and which pay toward Part B.

What this page can't tell you

  • Whether a plan's doctors and hospitals near you are in its network: check the plan's provider directory.
  • Drug costs. Each brochure has its formulary and drug tiers; most plans also enroll Medicare members in a Part D drug plan.
  • Your total for the year. That depends on your premium, whether you have Part B, how much care you use, and what your plan waives with Medicare. Our free FEHB/PSHB check tells you whether Part B is required for you and what it costs with IRMAA; the report then adds each plan's 2027 premium and cost-sharing into one yearly figure, with and without Part B.

Common questions

How much are FEHB premiums for retirees in 2027?

It depends on the plan and on Self Only, Self Plus One or Self and Family. Annuitants pay the monthly enrollee share in OPM's 2027 rate tables. Among the FEHB plans sold nationwide, the 2027 Self Only share runs from $149.58 a month (Compass Rose Health Plan: Standard Option) to $669.46 (SAMBA: High).

Do retirees pay the same FEHB premium as employees?

The enrollee share comes from the same OPM table. OPM states the government contribution for "most employees and annuitants" as one figure, so most retirees pay the same share as employees in the same plan, per month instead of per paycheck, taken from the annuity.

How much did FEHB and PSHB premiums go up for 2027?

OPM's 2027 Open Season Highlights give the averages: the enrollee share rises 10.9% in FEHB and 8.2% in PSHB (overall premiums 9.3% and 6.7%). Your own plan can differ a lot from the average in either direction.

Do I pay the Part B premium on top of my FEHB premium?

Yes, if you have Part B. It is a separate Medicare premium: $202.90 a month at the 2026 standard rate, more with IRMAA. CMS had not published the 2027 Part B premium when we checked on November 1, 2026; it usually does in November.
